- Eligibility certification must be completed
- Reimbursements can be request by mail
The simple act of saving a few dollars is usually enough to make consumers feel confident about a purchase. This same concept even applies to necessities such as prescription medications. Those who have been prescribed Bystolic and don’t mind the idea of saving a few bucks can now activate or register for a Bystolic Savings Card. To begin the activation or registration process patients will need to go to the Bystolic Savings page and click either the Activate Your Card button or the Register for a New Card button. To begin either process patients must first certify their eligibility by confirming whether they are a United States or Puerto Rico resident, whether they have commercial prescription drug insurance, and whether they are 18 years of age or older. To complete the eligibility certification patients must check a box agreeing to the certification and the program Terms, Conditions, and Eligibility Criteria available on the registration page or on back of the card.
What should I submit if I get my prescription through a mail-order pharmacy?
- A photocopy of both the front and back of the Bystolic Savings Card
- A photocopy of the insurance card (both front and back are needed)
- The original proof of purchase along with the patient’s date of birth
Patients will need to send the required information needed to request reimbursement directly to the Bystolic Claims Processing Department. The original proof of purchase should include an original pharmacy receipt with a name and address, pharmacy name, product name, prescription numbers, NDC number, date filled, quantity, and price. Once the needed information has been mailed patients can expect to wait 6-8 weeks to receive reimbursement. Patients should make sure their reimbursement requests are post-marked within 4 weeks of the fill date.
