The Health Choice service is funded by the tax payers both federal and state. Any questions about the service and or policies associated with Oklahoma Insurance Benefits can be directed to 3545 NW 58th St., Suite 110, Oklahoma City, OK 73112. Questions about the Health Choice OK portal can also be directed to HealthChoice Member Services @ 1-800-752-9475.
H.E.L.P Program Requirements
- Placing attention on face-to-face interaction between the member and their primary care physician
- Using preventive medicine in addressing health concerns
- Promoting wellness by making members aware of health risks related to lifestyle choices
- Supporting initiatives that work to improve the overall health of participating members
Preventive Services Highlights and Lowlights
- Routine exams for both men and women (the number of exams allowed per year at no cost will depend of the person age, sex and health)
- Common immunization as defined by the Centers for Disease Control and Prevention to include shots for Hepatitis A, Japanese Encephalitis, Tetanus, Diphtheria, Pertussis, Measles, Yellow Fever and the Flu just to name a few
- Consumers who use non network providers may be charged a fee
- Please note Vaccines (AKA serums) are required to come from a recognized health provider to be covered by HealthChoices OK
